Directions: From I-94 exit 159, go north on M-52 one mile; turn right at the second light, onto Will Johnson Dr. to enter hospital campus. Chelsea Community Hospital, a 113-bed nonprofit, acute care facility on a 119-acre wooded site, offers a wide range of surgical and clinical services.

Behavioral Health Services. 475-4029, (800) 328-6261. Inpatient and outpatient mental health services for adults, including inpatient detox. Partial hospitalization and intensive outpatient programs. The Older Adult Recovery Center (475-4030) offers outpatient detox and therapy for older adults. www.cch.org/oarc

Business Health Services. 475-4140. Offers customized programs for business and industry, including urgent care and follow-up of work-related injuries, and a variety of cost-effective screening and prevention programs for employees.

Cardiac/Pulmonary Rehabilitation. 475-4016. Services include complete cardiac screening exams, individually monitored exercise sessions, and rehabilitation programs to increase cardiac fitness and improve cardiovascular conditioning.

Cardiopulmonary Services. 475-3951.Comprehensive diagnostic services and treatment for cardiac, pulmonary, and vascular disorders, including stress tests, echocardiograms, EKG's, and EEG's. Other services include arterial doppler testing of the lower extremities and cardiac or pulmonary rehabilitation.

ChelseaCare. Provides a variety of comprehensive health services in your home through certified home health care (475-4190), pharmacy and home medical equipment (475-3996), private nursing (475-4190), and Lifeline services (475-4074).

Chelsea Orthopedic Specialists. 475-4028. Services include sports medicine, general orthopedics, total joint replacement, and hand, wrist, shoulder, foot, and ankle surgery.

Chelsea Rehab. Services include: hand rehabilitation (475-3947), neuro rehab program (475-4138), occupational therapy, physical therapy (475-3947), aquatic (pool) therapy (475-3947), lymphedema and cancer care (475-4085), orthopedic rehabilitation (475-3947), speech therapy (475-4138), and recreational therapy (475-4138).

Community Health Services. 475-4103.

Congregational Nurse Connection. 475-3992. A collaboration with area religious congregations to promote health. Identifies needs and assists in finding resources for care.

Diabetes Education. 475-4103. Diabetes nurse educators and dietitians assist in developing personalized plan. Group and individual sessions are offered.

Endoscopy. 475-3962. Procedures include colonoscopy and flexible sigmoidoscopy, gastroscopy, and esophageal dilation. 528-5381 (Huron Gastroenterology) or 936-9250 (Digestive Health Center at University of Michigan Hospital).

Healthy Communities Initiative. 475-6121. Promoting exercise and healthy diets to combat chronic diseases, particularly those associated with being overweight. The Walking Program is a structured regime for people of all activity levels. www.cch.org/healthycommunities

HeadPain Treatment. 677-6000. Comprehensive inpatient program for the treatment of patients with severe, debilitating, intractable head, neck, and facial pain.

Imaging Center. 475-3970. Bone density testing, CT and MRI scans, radiology, digital mammograms, ultrasound, and nuclear medicine.

Intensive Care and Cardiac Care Unit. 475-1311.

Laboratory services. 475-3932. Services include an outpatient blood drawing station, in-home blood drawing services for homebound patients, a blood bank, laboratory medicine, and pathology services.

Pain Clinic. 475-4106. Procedures for patients with chronic and acute pain, including epidural injections, nerve blocks, and advanced technologies, such as radio frequency generator (RFG), ablation of nerves, implantation of neurostimulators and pain pumps, disk ablation procedures, and others.

Rehabilitation Services. 930-0201, (800) 828-8020. Occupational and recreational therapy (475-4085), aquatic therapy, speech and language therapy, physical therapy (475-3947), cancer care (475-4085), back and neck disorders (475-3923), stroke, and brain injury (475-4138).

Sleep Disorders Clinic. 475-3963. Offers overnight sleep monitoring and treatment for patients with sleep apnea, narcolepsy, and restless leg syndrome.

Social Work Services. 475-3952.

Surgery Center. 475-3915. 11,000 surgeries performed annually in gynecology, orthopedics, opthalmology, neurosurgery, podiatry, and spine, plastic, and general surgery. Pain relief and endoscopy.

Therapy Services. Services include: hand rehabilitation (475-3947), neuro rehab program (475-4138), occupational therapy, physical therapy (475-3947), aquatic (pool) therapy (475-3947), lymphedema and cancer care (475-4085), orthopedic rehabilitation (475-3947), speech therapy (475-4138), and recreational therapy (475-4138).

White Oak Inn. 475-4181. Provides lodging for families of hospital patients and community members.

Women’s Health Center. 475-3979. Annual exams, breast care, infertility workups, family planning, surgery, and hormone replacement therapy.

X-Ray/Advanced Diagnostic Imaging. 475-3970. Bone density testing, CT and MRI scans, radiology, digital mammograms, ultrasound, and nuclear medicine.

Members may use 25-yard lap pool, indoor track, cardio and weightlifting equipment, cycling studio, whirlpool, warm-water therapy pool, sauna, and steam room. Classes available in yoga, Pilates, aerobics, aquatics, cycling, and arthritis management. Massage therapy and nutrition counseling. Medically integrated services include fitness assessment, cardiac and pulmonary rehab, orthopedics, and physical and occupational therapy. Memberships and child care available. The center is home to both primary and specialty medical services to pediatric, adult, and geriatric patients. Family physicians and general internists provide routine office care as well as gynecologic, obstetric and well-baby care; minor outpatient procedures (vasectomy, proctosigmoidoscopy, and endometrial biopsy); immunizations; nutritional counseling; and hearing and vision screening. Specialty care includes cardiovascular, gastrointestinal, sports medicine, general surgery consultations and behavioral health.
